Polterghast is a post-Moon Lord boss. Defeating it unlocks a large selection of content, including the final tier of the Acid Rain, weapon drops from major Abyss enemies, and items crafted with Ruinous Souls.
Spawn
If Polterghast has not yet been defeated in the world, it will spawn after killing 30 Phantom Spirits, forewarned by the status messages "Wails echo through the dilapidated dungeon halls..." at 10 kills and "Long-dead prisoners seek their zealous revenge..." at 20 kills.
Polterghast can also be summoned by using a Necroplasmic Beacon in the Dungeon. This becomes the only way to summon the boss after defeating it for the first time, as it will no longer spawn via Phantom Spirits.

Aftermath
- A status message appears which reads "The souls released stir the acidic storms...", indicating that the third tier of the Acid Rain and all of its associated content are now available.
- A status message appears which reads "The abyssal spirits have been disturbed.", indicating that several Abyss enemies gain new item drops:
- The Reaper Shark can now drop Reaper Teeth, the Valediction, and Deep Sea Dumbbell.
- The Eidolon Wyrm can now drop the Eidolic Wail, the Soul Edge and the Eidolon Staff.
- The Colossal Squid can now drop the Calamari's Lament.
- The Bobbit Worm can now drop the Bobbit Hook.
- Dungeon Crates can now contain Phantoplasm, and Sulphurous Crates can now contain Reaper Teeth if The Old Duke has also been defeated.
- The Cyborg will begin selling the Lion Heart.
